My job is not to impose my aesthetic on other persons. This lovely woman came to me after having many operations and recurring problems, including capsular contracture and rippling, as seen here. She had implants that vastly overwhelmed her anatomy, which were contributory to many of her problems. Her tissues were thin, and in some areas, extremely thin. Her right breast was contracted and malpositioned, and her left breast had severe rippling, mild contracture, and an extraordinarily thin soft tissue envelope in the inner lower pole. We developed a plan that included bilateral total capsulectomies, placement of a biologic material known as acellular dermal matrix or ADM, and replacement with highly cohesive silicone gel implants. The ADM is a sheet-like material that I use to cover the part of the implant that isn’t under the pec muscle, which also makes it a useful sling to support the implant and provides a small addition to tissue thickness. Its main purpose though is reduction in recurrent capsular contracture. One of my main concerns in performing this operation was maintaining adequate blood supply to the skin overlying the implant in the lower pole of the left breast. After the operation, it was clear that that skin was experiencing suboptimal oxygenation; fortunately, as part of the plan, we used hyperbaric oxygen therapy (HBOT), both before and multiple times after the operation to help keep that tissue alive through the healing process and lower the risk of infection. Tissue necrosis or infection would have been catastrophic had it happened. This was a very risky operation that turned out well.
